In an effort to provide seniors with easily accessible healthcare service without limiting their mobility, the first Mobile Integrative Health Centre (MIHC) in Hong Kong was launched in January 2008. In this session, Prof Chung will to implementation and discuss the impact of the MIHC on Hong Kong’s senior population in this session.

The implementation of a knowledge driven and easy to use documentation solution is challenging. This presentation explores a model for designing clinical documentation so it enhances workflow, streamlines communication, improves decision support, and increases patient safety. A framework that can be generalized to a variety of settings will be presented.
