As organizations move towards the road towards electronic medical record, implementation has taken to chasing what’s new and what’s the perfect solution. However, many fail to take a step back and review the basics. We will trace the path of the fundamental of nursing documentation as it moves away from paper and the lessons learnt.

Nursing Documentation is very crucial as it concerns the quality of care rendered to our patients. Rise in litigation may occur if documentation is not done or is incomplete. Challenges were encountered in nursing progress notes, nursing notes and nursing care plan (NCP) notes. NCP committee was formed and challenges were met by creating new NCP based on activity daily living, available in the system via hyperlink, on going training and continuous surveillance. Resulted in 100% sisters and staff nurses being trained to these new NCP and challenges were met progressively and effectively.

Innovation in Medication Administration: The Hospital Putrajaya Experience

This case study describes the problem of transcription of doctor's prescription that allows medication errors, poor medication administration and documentation, which in turn compromises patients’ safety. A pilot study was done from 1/5/2005 to 30/10/2005 in 3 surgical wards and there was much improvement after remedial actions done. Hospital wide implementation of the change was carried out from 2007 to 2008. For continuous monitoring and trending, a re-audit was done in December 2008 to study the magnitude of improvement.

Victoria M. Bradley RN, DNP, FHIMSS, CPHIMS
Chief Nursing Informatics Officer
Eclipsys Corporation, USA

Victoria M. Bradley is the Chief Nursing Informatics Officer at the Eclipsys Corporation. She works closely with clients to determine priorities for solutions that support the clinical and administrative practice of nursing.  She also provides guidance in the development of these nursing solutions.  Currently Dr. Bradley also serves as Assistant Professor, adjunct faculty for the University of Kentucky, College of Nursing. And she is president elect for ANIA, the American Nursing Informatics Association.

Previously she was Director of Patient Information at the University of Kentucky HealthCare, where she was project director for implementation of an enterprise wide integrated clinical information system and administratively responsible for the hospital medical records department. Her experiences include system selection, contract negotiation, implementation and evaluation.

Dr. Bradley was active for many years in the specialty of emergency nursing as an educator, director, speaker and author.  She is a past president of the Emergency Nurses Association and the Emergency Nurses Foundation.  She has received the Pillar Award of the ENA Foundation in 1999 and the Judith C. Kelleher Award in 1996 from ENA. 

Park Hyeoun-Ae RN, PhD
Professor, Biostatistics and Health Informatics, College of Nursing,
Seoul National University, Korea

Dr Park is a Professor of biometry and health information systems with the Seoul National University College of Nursing in Seoul. She earned a PhD degree in biometry and health information systems. She is a Vice President for WGs and SIG of the IMIA, and a member of ICNP Strategic Advisory Group of the ICN. She also actively participates in the ISO/TC 215, a technical committee on Health Informatics standards development. She spent a year at the SNOMED Terminology Solutions focusing on the development of principles, processes, and strategies to integrate, map, and/or model ICNP concepts within SNOMED CT.

Yap Soon Ghee RN, BN, GradDipClinNsg (EENT)

Soon Ghee works with the Nursing Division of Singapore General Hospital on Nursing Informatics. He was trained in the field of Opthamology and ENT nursing and has worked on numenous projects in healthcare informatics in Australia, China and developing countries. He is currently working on the implementation of CPOE, medication administration and nursing documentation within Singhealth. He is currently researching the area of computer user satisfaction among nurses.

Madam Zaina Mohamad

Matron Zaina has been in nursing service for the past 33 years. Her achievements include post basic in Midwifery, Orthopedic, Nursing Management and Degree in Nursing Sciences in 1996. Her nursing experiences has taken her to many places in the country including Hospital Kuala Lumpur. She has served at the Malaysian Nursing Board as a secretary for 5 years, and also as a state Matron of Selangor. Currently she is in Selayang Hospital since 2006

Madam Leong Foong Khuan

F.K. Leong is the nursing director in Putrajaya Hospital since 2003. She has 32 years of nursing experience. Her achievements include post basic training in Midwifery, Personnel Management in healthcare services and Central Sterile Supply Technique. She was an active Infection Control Nurse for 10 years and has set up the infection control teams in a few hospitals in the country. Her experiences include wound care management and Quality Assurance Program. Currently she is in her last year of B.SC (Hons) In Nursing Practice Development.
